Because I help my clients learn to navigate effectively in their professional environment, I first need to know about their important relationships.
If you're one of my coaching clients, you'll recognize this form as the first thing I ask you to complete when we begin our work together.

This form helps you to get a better idea of how supportive (or not) the key persons are who are most important to you. You should fill out a separate form for each person you wish to analyze. Most people pick their boss, a couple of peers, and a few direct reports.
The analysis leads you through a couple of ways of looking at an individual. You end up making an action plan identifying a few key actions which YOU can take that in your opinion if done, will make it more likely that the key individual will be more positive and help you achieve what you want.
Note: this is not about actions they can do, instead it's about things you can do to improve your chances that they will be supportive. It might be as simple as realizing that you need to get on your boss's calendar and explain what the benefit is if they approve your budget for a new idea. You'd then lock in your commitment to get the appointment by a certain date. This way you can make certain you keep on track doing the things you've agreed will help you to influence others to be more likely supporters of you.
With each individual person's form you'll track through the ways of looking at how the key person relates to you and your goals. You'll end up choosing several discrete goals in the action plan for each person.

What's On the Form
Here is a summary of the questions I ask to help me understand key relationships in your professional environment:
For each key business relationship:
- In general, do I feel positive or negative after interacting with this person?
- How much time do I spend with this person?
- How much impact does this person have on me?
- What behaviors affect me feeling positive or negative after interacting with this person?
- What is the degree of change difficulty and payoff?
In our coaching session we lay out the information above, and set a goal based on what we identify as the low-hanging fruit: people who would have a higher payoff if they became more positive towards helping you achieve your goals.
From there we develop action plans, including specific things you want or need from this person. Ask yourself:
- What will you achieve if the goal is met?
- What specific action can you take with the person to help the goal occur?
Then commit to a specific date by which you will take action.
